Warranty Conditions

To qualify for warranty coverage:
  1. The claim must involve the original GDT implant and original purchaser.
  2. The warranty is limited to the replacement of the product only.
  3. The claimant must provide appropriate supporting documentation, which may include:

Clear photos of the implant and packaging

Relevant clinical documentation describing the issue

4. A fully completed complaint report must be submitted for each case.

5. No other warranties, expressed or implied, exist beyond the conditions defined in this document.

Warranty Claim Procedure

If you believe a GDT implant case may fall under the lifetime warranty:
  1. Obtain the complaint report form: Available via your GDT representative or customer service.
  2. Complete the complaint report: Include product details, batch/lot number, and a clear description of the problem.
  3. Attach supporting evidence: Photos and clinical documentation showing the defect, fracture, rejection, or contamination.
  4. Submit the complaint: Send the full documentation set to GDT Dental Implants or your authorized GDT distributor.
  5. Product evaluation and decision: GDT will examine the product and review all submitted evidence. If the complaint is approved, a replacement implant will be issued according to the warranty terms.

Intended Use and Handling Requirements
